Volunteer Form

Thank you for considering to volunteer for CSA. We will be contacting you to discuss how you can best fit into our organization. Please note that for the safety of our children, all volunteers will have to submit an additional online application and will be subjected to periodic background checks, at a schedule set by Florida Youth Soccer Association, FYSA, using whatever services or methods that the FYSA deems appropriate. The results of this background check may be used to deny your participation with any FYSA affiliated organization or program.
